Intake Form – this is our starting point to learn about your disability claim.
Form 1696 – Appointment of Representative – we need to submit this signed form to Social Security to start work on your case
SSA Form 1693 – contingency fee contract provided to us by SSA – no fee unless we win; fee is 25% of past due benefits with a cap of $7,200
Form SSA-827 – SSA Medical Authorization – this is Social Security’s medical authorization form
Ginsberg Law medical authorization – this is the form we use when requesting records from your doctors
